Spa Pre-Delivery Checklist

Customer is responsible for preparing site prior to spa delivery.
Customer is responsible for the electrical hook-up of spa.
Customer is responsible for providing sufficient access to spa site prior to delivery.
Customer is responsible for any and all costs related to special equipment (i.e. crane) or extra labor needed for spa delivery.
Customer is responsible for removal of old spa.
I agree to all items as written on my accompanying spa order.
If constructing a new deck, check with delivery company before adding rail and/or steps.
A copy of all applicable labor costs is available upon request
